1. CMH Junior Club membership is applicable to both Malaysians and non-Malaysians from newborn to 12 years of age.
2. The membership will automatically expire on 31st December of the child’s 12th birth year.
3. Members are required to show the CMH Junior Club Membership Card to enjoy privileges and benefits given by CMH Specialist Hospital and its Participating Merchants. No reimbursement or refund will be made on previous or back-dated transactions if card-holders fail to present the Membership Card during registration.
4. Discounts at CMH Specialist Hospital are applicable for SELF-PAY customers and are NOT applicable on doctor’s fees and other on-going promotions, discounts and / or privileges.
5. A nominal fee of RM5 will be charged for a replacement card.
6. CMH Junior Club membership is non-transferable to a third party or applicable with any other promotions, discounts and/or privileges.
7. Should any of the conditions and privileges are misused or abused, CMH Specialist Hospital is entitled to suspend and terminate the member’s right to use the CMH Junior Club Member Privilege Card without prior notification.
8. CMH Specialist Hospital acts as a platform for its members to access the benefits and privileges offered by participating merchant(s). It’s important to note that the association between CMH Specialist Hospital and these merchants doesn’t establish a partnership, joint venture, or agency relationship. CMH Specialist Hospital holds no responsibility or liability for any issues arising from the products or services provided by these merchants. 9. The hospital shall not be held liable for any failure on the part of the member or guardian to claim benefits offered by our participating merchant(s). It is the responsibility of the member or guardian to adhere to the terms and conditions set forth by the participating merchant(s) in order to avail themselves of any benefits or discounts.
10. The hospital does not guarantee the availability, quality, or validity of benefits provided by participating merchants. Any disputes or issues related to the benefits offered by participating merchants should be addressed directly with the merchant(s) in question.
11. The hospital shall not be responsible for any financial losses, damages, or inconveniences incurred by the member or guardian as a result of their failure to claim benefits at our participating merchant(s), nor shall the hospital be obliged to compensate for any such losses or damages.
12. CMH Specialist Hospital and its Participating Merchant(s) reserves the rights to discontinue or modify any of the Terms & Conditions, benefits and privileges deemed necessary at any time without prior notification.
